SHIPPING

  • We offer same day pick up for cases within a 30 mile radius. Please call our office prior to 12pm to let us know that you have a pickup. If you are outside of our 30 mile radius, you are responsible for shipping to ODS.  We cover the cost of return shipping.

INVOICING AND PRICING

  • Invoices will be provided with every case as it is returned to the office.  Statements will be sent monthly.

  • Please contact the lab if you need an updated price list.

PAYMENTS

  • Payments are due on the 15th of the month following the invoice date. Statements not paid by the due date will incur a finance charge of 2% on the entire outstanding balance (24% per annum).

  • When a client’s past-due unpaid statement exceeds US $1000 in value ODS inc. retains the right to refuse service to that client and / or collect via C.O.D. plus 20% of remaining balance.

  • Balances not paid after 90 days will be turned over to a collection agency and the account will be placed on credit hold.

  • Each new shipment to ODS inc. constitutes a complete and separate transaction to be billed and collected. Acceptance of new orders by ODS inc.shall not relieve the customer of any previous indebtedness to the ODS inc. lab

QUALITY / WARRANTY

  • We stand behind all our restorations and will remake for any quality issue due to material break down.  Cracks and or broken teeth do not fall under this warranty.  A tooth that 'pops' out of an appliance because of lack of material / chemical bond will be replaced under the warranty.  We offer a 5 year warranty on Premium products and a 1 year warranty on all other products. 

  • ODS inc. will only remake what’s ordered on the original lab slip.

  • Original restoration must be included with the remake or it will be considered a new case.

  • ODS inc. will provide credit only after we have had the opportunity to do a remake.

  • If ODS inc. questions an impression, Digital file and the dentist approves completion of the case without resolution to the question, a 100% remake charge will be assessed if the restoration is returned.

REPAIRS, RESETS & ADJUSTMENTS

  • Resets in wax are free of charge.

  • Adjusting acrylic work is free of charge.

  • Changes or additions to the case will result in additional charges.  

  • We do offer Same-Day repair and Relines services.  Please call the lab for full details.

  • We can create an account for you to access our lab portal to view case status, request pickups and make payments.  Please contact the lab if you would like us to set this up for you.
